The GEN215B testing report is completed by the NYS Certified Backflow Prevention Device Tester. A Professional Engineer or Architect must be retained to sign off on the GEN215B form to certify that the installation was completed according to the approved NYC DEP plans. This could be your originally contracted Professional Engineer or could be a Professional Engineer hired by your NYC Licensed Master Plumber. Ensure this is determined prior to hiring your NYC Licensed Master Plumber.


The GEN 215B form is signed-off and submitted to the NYC DEP in order to achieve project close-out. Project close-out should be completed within 2 years of the original application approval date. If the GEN215B form is not signed-off and submitted to the NYC DEP, the project will not be closed-out and a violation could be issued with a maximum penalty of $1,000.
