NYC DEP Backflow Prevention Device Application and Approval Process:


1. Hire a Professional Engineer or Registered Architect such as Built Engineers & Architects. Call 646.481.1861 or email info@builtengineers.com


2. The plans and applications for the device are prepared by a Professional Engineer or Registered Architect and submitted to the NYC DEP. A $350 filing fee is required to be submitted to the NYC DEP for each newly-proposed device. Multiple devices may be required depending upon the number of incoming and combined water services in the building.


3. After submission to the NYC DEP, the Professional Engineer or Registered Architect will receive comments back on the plans and application. The documents are then revised and resubmitted to the NYC DEP for final approval.


4. Upon NYC DEP approval, the Professional Engineer or Registered Architect will receive the NYC DEP approved plans and application. The NYC DEP approval is valid for (2) years from the application approval date.


5. The new required devices are to be installed by a Licensed Master Plumber according to the approved plans.


The following installation prices (in addition to filing and violations removal costs) are estimated based on average industry prices for parts and labor:


  • For very small buildings (1 story) installation should cost between $3,000 and $5,000.

  • For small buildings (2 story) installation should cost between $3,750 and $5,500.

  • For small to mid-size buildings (car washes, laundromats, small manufacturers) installation should cost between $5,000 and $7,000.

  • For mid-size buildings (office complexes, large manufacturers or department stores) installation should cost between $7,500 and $13,000.

  • For large buildings (High rises, hospitals) installation should cost between $14,000 and $34,000.

6. After the devices are installed by a NYC Licensed Master Plumber, they must be tested by a NYS Certified Backflow Prevention Device Tester. Ensure this testing is included as part of your scope with your NYC Licensed Master Plumber.


7. The GEN215B testing report is completed by the NYS Certified Backflow Prevention Device Tester. A Professional Engineer or Architect must be retained to sign off on the GEN215B form to certify that the installation was completed according to the approved NYC DEP plans. This could be your originally contracted Professional Engineer or Architect or could be a Professional Engineer or Architect hired by your NYC Licensed Master Plumber. Ensure this is determined prior to hiring your NYC Licensed Master Plumber.


8. The GEN 215B form is signed-off and submitted to the NYC DEP in order to achieve project close-out. Project close-out should be completed within 2 years of the original application approval date. If the GEN215B form is not signed-off and submitted to the NYC DEP, the project will not be closed-out and a violation could be issued with a maximum penalty of $1,000.
